Congratulations!  You signed up for an innovative model that is actively working to fix our broken healthcare system and rebuild the patient-physician relationship.  We believe that everyone has the right to a physician who knows them and can care for them like family.  Board-certified family physicians can offer comprehensive primary care covering 90% of healthcare needs.  We believe DPC is best paired with a high deductible insurance plan as unepected events do happen.  When you need a specialist or additional services, Fulcrum Family Health will work with your insurance to coordinate your care.  If you don't have insurance, we will do our best to navigate your care considering cost and quality. We hope you enjoy your direct primary care experience.  â€‹

Next steps

  1. Confirmation of Membership

    • Your membership is unconfirmed until you speak with our staff.  We will call to ensure there are no issues with your account in our system including start date and contact information.​

  2. Schedule your initial appointment

    • ​Our staff will reach out or you are welcome to call at your convenience.  We will help you make your first appointment.  After you are established, you can take advantage of our self-scheduling, if you'd prefer.​​

  3. Intake Forms and Prior Records

    • You will receive an email 2 days before your initial visit with several questions about your history.  This information helps fill out your medical record.  Please answer to the best of your abilities. You will also see a release of information (ROI) form.  We can send this to the designated medical office and retrieve your records.  Of note, MaineHealth and Intermed will only send records if you sign their specific form.  In those cases, we ask you to reach out to them and use our information seen below.  Alternatively, you can print out the "Paper Release Form" seen below, complete and bring those to your appointment.  â€‹

  4. Communication

    • At your first appointment, your physician will review communication and online scheduling platforms and explain how best to contact us.  If you have any questions before your first appointment, please contact our front desk by email or phone number.
